What is needed to enroll a student - all need a proof of residence (lease, property tax bill, utility bill)

• Pre-Kindergarten
• Kindergarten
• From any other school: consent to release records, enrollment form
• From non-CCPS school: also need birth certificate & Maryland Home Language Survey
• From non-Maryland: also need Maryland Schools Record of Physical Examination
